  • Bronchoscopy vs EBUS: What Is the Difference?

You may have been told you need a bronchoscopy or an EBUS and are wondering: what is the difference, and which one do I need? As a respiratory investigation specialist trained in both procedures, let me explain in straightforward terms so you feel fully prepared.

Standard Bronchoscopy: What It Is

A standard bronchoscopy uses a thin, flexible tube with a camera to look directly inside your airways. I can see the inside lining of the airways, identify inflammation, irritation, mucus, or blockages, and take biopsies from visible areas. Think of it as looking through a tunnel — I can examine everything inside the tunnel walls in detail.

EBUS: Bronchoscopy With Ultrasound

EBUS stands for Endobronchial Ultrasound. It is a specialised type of bronchoscopy where the scope has an ultrasound probe on the end. This allows me to see everything a standard bronchoscopy shows plus structures outside your airways — lymph nodes, masses, and tissue beyond the airway walls. Think of it as having X-ray vision through the tunnel walls to what lies beyond.

When Standard Bronchoscopy Is Used

Standard bronchoscopy is used when the problem is clearly inside your airways. Common indications include a persistent cough, coughing up blood, suspected airway infection, or an abnormal finding on a chest X-ray that needs direct visualisation and sampling.

When EBUS Is Recommended Instead

EBUS is recommended when we need to sample lymph nodes or tissue outside the airways. This includes suspected or confirmed lung cancer requiring staging, enlarged lymph nodes found on a CT scan, sarcoidosis diagnosis where lymph node sampling is needed, and unexplained masses near but not inside the airways.

What Is the Experience Like for the Patient?

Honestly, the experience is very similar for both. Both procedures use local anaesthetic throat spray and offer sedation. Both take around 20 to 30 minutes and have similar recovery times. EBUS takes slightly longer because I am taking samples from additional areas, but patients typically do not notice a significant difference between the two.

Accuracy and Diagnostic Results

Standard bronchoscopy is highly accurate for airway problems. EBUS has over 90% diagnostic accuracy for lymph node involvement — far superior to older surgical methods and significantly less invasive. This level of precision means the correct treatment plan can be put in place from the outset, without unnecessary repeat procedures or surgery.

Why My Training in Both Procedures Matters for You

I completed advanced fellowship training in interventional pulmonology in Italy and regularly perform both standard bronchoscopy and EBUS. This means I can select the most appropriate procedure for your specific situation, achieve accurate diagnosis at the first attempt, and access the most advanced diagnostic techniques without referring you elsewhere.

How We Decide Which Procedure You Need

Your CT scan results usually indicate which procedure is most appropriate. Standard bronchoscopy is used when the problem is clearly inside the airways. EBUS is selected when lymph nodes or tissue outside the airways need sampling. Occasionally, I begin with a standard bronchoscopy and add EBUS during the same session if findings indicate it is needed.

Key Points to Know

  • Standard bronchoscopy examines the inside of the airways
  • EBUS adds ultrasound to see lymph nodes and tissue outside airway walls
  • Both use local anaesthetic and sedation — the patient experience is similar
  • EBUS achieves over 90% accuracy for lymph node diagnosis
  • CT scan results usually determine which procedure is most appropriate

Frequently Asked Questions

Is EBUS the same as a bronchoscopy?

EBUS is a specialised form of bronchoscopy. It does everything a standard bronchoscopy does but adds an ultrasound probe, allowing tissue sampling from lymph nodes and structures outside the airways.

Which is more accurate — bronchoscopy or EBUS?

For airway problems, both are highly accurate. For lymph node assessment, EBUS is far superior, achieving over 90% diagnostic accuracy compared to older surgical methods.

Is EBUS more painful than a standard bronchoscopy?

No. Both procedures use the same local anaesthetic and sedation. EBUS takes slightly longer, but patients typically do not experience any meaningful difference in comfort between the two.

How long does EBUS take?

The EBUS procedure takes approximately 20 to 30 minutes. Including preparation and recovery, patients should expect to be at the clinic for around three to five hours in total.

Do I need EBUS or a standard bronchoscopy?

This is determined by your CT scan results and clinical picture. Dr Suman Paul will review your imaging and recommend the most appropriate procedure during your initial consultation.
