As a respiratory consultant treating COPD patients across Liverpool and Merseyside, I regularly teach these techniques to help patients manage their symptoms more effectively. Breathing exercises can help by improving lung efficiency, reducing breathlessness during activities, helping empty trapped air from the lungs, strengthening breathing muscles, and reducing anxiety during breathlessness episodes.
1. Pursed Lip Breathing
This is one of the most effective breathing techniques for COPD patients. Breathe in slowly through your nose for two seconds. Purse your lips as if you are about to whistle. Breathe out slowly through pursed lips for four seconds — your exhalation should be twice as long as your inhalation. Pursed lip breathing helps keep airways open longer, release trapped air from the lungs, and reduce breathlessness during activity. It is particularly useful when climbing stairs, walking uphill, or exercising.
2. Diaphragmatic (Belly) Breathing
This exercise trains your diaphragm — the primary muscle used for breathing. Many COPD patients rely too much on chest and shoulder muscles, which makes breathing less efficient. Sit comfortably or lie down. Place one hand on your chest and the other on your stomach. Breathe in through your nose so that your stomach rises while keeping your chest as still as possible. Breathe out slowly using pursed lips. Practise this exercise for 5–10 minutes daily.
3. Blow As You Go Breathing
This technique helps manage breathlessness during physical effort. Breathe in before the activity and breathe out during the effort. Examples include standing up from a chair, climbing stairs, and lifting shopping bags. Exhaling during the effort helps reduce pressure in the lungs and prevents breath holding, making everyday tasks significantly more manageable.
4. Paced Breathing for Walking
Walking can be challenging for many COPD patients, especially on hills or uneven ground. Paced breathing helps control your breathing rhythm while walking. Walk at a steady pace. Breathe in for two steps. Breathe out for four steps. Adjust the step ratio to match your comfort level. This technique helps you walk longer distances with less breathlessness.
5. Recovery Breathing Position
When breathlessness becomes severe, body positioning can help improve airflow. Standing recovery position: lean slightly forward, rest your hands on your knees or a wall, relax your shoulders, and use pursed lip breathing. Sitting recovery position: sit and lean forward slightly, rest your elbows on your knees or a table, and focus on slow breathing. Both positions allow the diaphragm to work more effectively.
How Often Should COPD Breathing Exercises Be Practised?
For best results, breathing exercises should be practised regularly. Respiratory specialists typically recommend 10 minutes twice daily when relaxed, using techniques during physical activity, and practising before breathlessness becomes severe. Consistency helps improve breathing efficiency over time and makes the techniques more natural to use when you need them most.
What Is Pulmonary Rehabilitation?
Breathing exercises are often included in pulmonary rehabilitation programmes — a structured programme that combines breathing techniques, supervised exercise training, education about lung disease, and lifestyle support. Research shows pulmonary rehabilitation can improve exercise capacity, reduce breathlessness, lower hospital admissions, and improve quality of life. Many programmes are available across Liverpool, Merseyside, and Greater Manchester.
When Breathing Exercises May Not Be Enough
While breathing exercises are very helpful, persistent or worsening symptoms should still be evaluated by a specialist. You should consider seeing a respiratory specialist if you experience daily breathlessness affecting activities, frequent chest infections, increased use of reliever inhalers, worsening COPD symptoms, or limited improvement despite treatment. Specialist evaluation can help optimise medications and management strategies.
